How to Get Military Medical Records

Use the “Request Military Records” button to launch the application tool. Fill out the personal information in the first two steps. Choose “Medical” for the category of your request in the second step. You will then receive three options for medical records: outpatient treatment records, inpatient treatment records and mental health clinic records. Choose only one of these even if you are requesting all of your Air Force medical records. You can expand your request later in the application process.

Making An Access To Information Or Privacy Request

We are subject to the Privacy Act and Access to Information Act.

The Privacy Act guides the collection, retention, and release of personal information by the Government of Canada. It also gives Canadians the right of access to information about them which is under the control of a federal government institution. This right is subject to specific exemptions.

Library and Archives Canada is responsible for holding the service records of Regular Force members who released before January 1998 and Reserve Force members who released before January 2007. Theyre also responsible for the medical and dental records of Canadian Armed Forces members released more than 5 years ago or who died in service more than 5 years ago.

Apply For Your Own Records

You can apply for your own records by completing a Subject Access Request form if you are, or have been, a member of the armed forces, for example:

  • the Royal Navy
  • the British Army
  • the Royal Air Force

Send it to the address on the form, along with any supporting documents. Theres no fee.

Also use this form if youre acting on behalf of the person, for example if you have lasting power of attorney.

Who Can Obtain Veteran Records

  • Veterans. Recently-separated veterans can access their own records online from the eBenefits web site, including DD 214s, NGB-22s, AB8 Benefit Verification Letter, and more. More information is available in this eBenefits Fact Sheet. For older records, veterans can complete an online form at eVetRecs, but the request must then be printed out, signed. And mailed in.

How To Request Nprc Records

NPRC records can be requested by online using eVetRecs, by using Standard Form 180, Request Pertaining to Military Records, or by sending a letter .

To request records pertaining to time spent in combat, transport between assignments, or hospitalization requires that specific details be provided so that the records can be properly located. For example, tell the NPRC the name of any hospital where you were admitted for treatment during service or indicate in detail where you served in combat.

Records From A Provider No Longer In Practice

Sometimes, patients may need records from a doctor no longer in practice. Under law, the doctor must transfer his or her records to another provider.

If the doctor left a practice that is still operating, the records will remain with the practice. If another doctor bought the practice, the new practice would maintain the records.

If the provider didnt leave any details before leaving practice, a patient may be able to put together records from hospitals, labs and specialists they saw. The insurance company may also be able to help.

Who May Request Medical Records

While designed to protect your privacy, HIPAA regulations are so extensive that many providers are still confused about how to enforce them. This can sometimes make it difficult to obtain your records, even if you are fully entitled to them.

According to HIPAA, you have the right to request medical records in these circumstances:

  • You are the patient or the parent or guardian of the patient whose records are being requested.
  • You are a caregiver or advocate who has obtained written permission from the patient. In some cases, the healthcare provider will provide you a permission form that the patient must complete.

Many people assume that only they or their designees can obtain copies of their medical records. Under the law, there are other individuals or organizations who may also have the right.

Help Your Gp Access Your Military Medical Records

When you first visit a GP at your NHS GP surgery, it is important to give them the medical summary form you received when you were discharged from the armed forces. This form includes the details needed for them to request your complete military GP medical records from DMS.

Once you have given them your form, you can check with a GP at a later date to make sure your full medical records have been requested.

It usually takes several months for your GP surgery to receive your military GP records.

You can also give them a paper copy of your summary medical records if you got one during your discharge period. They might need this if they do not have access to your full military medical records yet.

What Information Is Needed To Request Military Medical Treatment Records

To obtain records from the NPRC, you are going to need to provide the following information about the Veteran:

  • The patients full name used during treatment
  • The patients Social Security number and status during treatment
  • The name of the last facility responsible for the treatment record, usually the last facility that provided treatment
  • The year and type of treatment, including type of illness or injury
  • The branch of service and sponsors service number or Social Security number

You want to provide as much information as you can because a record may not be located if information is missing.
